The 'My Number' social security system / 「マイナンバー」制度について

The Japanese government has adopted a new social security and tax number system, 'My Number Card (Individual Number Card)' in order to (1) enhance the social security to people who truly need it, (2) enhance public convenience and (3) develop the efficiency in administration.

My Number Card applications / マイナンバーカードの申請

Application Methods

To obtain a My Number Card, you can choose from a variety of application methods for your convenience. Please prepare your ID and the application form with QR code that you received in the mail (Individual Number Notification Card) to complete the process.

1) Via online (Japanese site)
You can apply from home quickly, if you have a proper self-portrait photo on your device.
2) Via a photobooth installed by various manufactures
You can take a proper photo of you and apply at the same time.
(List of manufactures' website is shown in the middle of this page)
3) By mail with a written application form
By using the partially-filledout application form, you can follow English-translated guide, and send in the documents in a return envelope enclosed with the Notification Card. It might be the easiest way for those who don't have enough confidence in Japanese ability.

4) By visiting Matsudo My Number Card Center

5) By visiting temporary My Number Card application support centers (Japanese site) and Japan Post Offices (JP Bank)
Matsudo city is offering My Number Card application support at various commercial facilities across the city for a limited time. Please see this website (Japanese) for the list of facilities and when they are holding the on-site application support.
My Number Card application is also supported at all Japan Post offices throughout the city with a prior reservation.

Receive your My Number Card

  1. Approximately one month after applying for your My Number Card, the city will mail a “My Number Card Issuance Notification” to your home address.
  2. Choose a pickup location and time to receive your My Number Card.
    Please have the “Login Notification” enclosed with your “My Number Card Issuance Notification” ready, and make a reservation via Online Reservation System, or by calling Matsudo City My Number Call Center (047-366-8178).
  3. Visit the designated location on the scheduled date and time. After your identification is confirmed, you will be asked to set TWO personal identification numbers; A 4-digit card PIN, and a 6 to16 alphanumeric characters for the electric certificate for the bearer's signature. Then you will receive your card.

■ Items to bring when picking up My Number Card

  • My Number Card Issuance Notification
  • Notification Card (if you have one)
  • Identification document (one of the following): Driver's license, passport, Residence Card, Special Permanent Resident Certificate, etc.

You can now receive your My Number Card at home

Under certain conditions, you may choose to have your My Number Card delivered to your home.

■ Elgibility for home delivery of the My Number Card

  • Applicant is applying for a My Number Card for the first time.
  • The applicant him/herself must visit the Matsudo My Number Card Center in person to apply. (No reservation is required.) Those under 15 years old must be accompanied by a parent/guardian.
  • All procedures must be completed at the time of application at the counter, including having your photo taken (free of charge), submitting your PINs, and verifying your identity.
    [Please bring one of the following forms of identification]
    (1) A + Notification Card(2) B + B + Notification Card
    (3) A + A(4) A + B
    A: Government-issued photo identification

    B: Other identification
    with “Name and Date of Birth” or “Name and Address”

    After application, your My Number Card will be mailed by registered mail to your registered address within approximately one month.

※ If any required documents are missing at the time of application, only the application form will be processed. Mail delivery is not available; you must return to the center in person to receive the card at the time of issuance.

Regarding representative pickup

In principle, the My Number Card must be picked up in person by the applicant him/herself. If the applicant is unable to visit the office due to unavoidable circumstances, they may authorize a proxy to pick up the card. However, additional documents and procedures are required.

Resetting the PIN for your My Number Card / マイナンバーカードの暗証番号再設定

If you have forgotten the PIN you set for your My Number Card or have entered it incorrectly multiple times in a row, you will need to reset your PIN.
*Note: The PIN will be locked if you enter the user authentication certificate PIN (4 digits) incorrectly 3 times or the signature certificate PIN (6-16 digits) incorrectly 5 times.

Method 1: Reset your PIN by visiting the service counter

Bring your My Number Card and visit one of the following service counters. There is no charge.

Method 2: Reset your PIN using a multi-function copier at a convenienve store

You can reset your PIN at convenience stores nationwide.
Please see the JPKI* website for a list of cooperating convenience stores that provide My Number Card PIN resetting services.

(* "JPKI=Japanese Public Key Infrastructure" is a public personal authentication service operated by the Local Government Information Systems Organization, a corporation jointly managed by the national government and local governments.)

■ Items required

Case [1]: When your signature certificate PIN is lost/locked (but you still have the user authentication PIN)
Case [2]: When your user authentication certificate PIN is lost/locked (but you still have the signature certificate PIN)

  • My Number Card
  • Smartphones with cameras capable of reading the IC chip on My Number Cards with the “JPKI PIN Reset app (IOS / Android)" installed.
  • [1] User authentication certificate PIN (4 digits) when resetting the signature certificate PIN
  • [2] Signature certificate PIN (6-16 digits) when resetting the user authentication certificate PIN
※If you do not know either PIN, you cannot reset it at a convenience store.

■ Steps

STEP 1: Preliminary registration on smartphones
  1. Open "JPKI PIN Reset app" on your smartphone
  2. Follow the on-screen instructions to photograph the front side of your My Number Card
  3. Scan the My Number Card and enter [1] the user PIN (4 digit numbers) or [2] the signature certificate PIN (6-16 alphanumeric characters)
  4. Proceed with facial recognition
  5. Complete the pre-registration
STEP 2: Operations at a convenience store
  1. Within 24 hours of making your registration, use a multi-function copier at a convenience store and select “Administrative Services”
  2. Select “My Number Card Password Reset”
  3. Select [1] "Reset/initialize the signature certificate PIN" or [2] "Reset/initialize the user PIN"
  4. Place your My Number Card on the reader and enter the [1] user PIN (4 digit numbers) or [2] signature certificate PIN (6-16 alphanumeric characters) for user authorization
  5. Enter the new [1] signature certificate PIN or [2] user PIN
  6. Finish the resetting procedures

If you lost your My Number card / マイナンバーカードの紛失・一時停止解除

If your My Number Card is lost or stolen

Please call the My Number Card General Toll-Free Number (0120-95-0178) as soon as you can to temporarily suspend the card's functions.
Additionally, if your My Number Card is lost or stolen outside your home, please report it to the police station.

Reactivation of suspended My Number Card

If your lost My Number Card is found, you must visit the My Number Card Center, the City Hall Resident Administration Division, or any branch office to complete the procedure to lift the suspension.
Please bring your My Number Card.
Note: If you have suspended your My Number Card, the electronic certificate for signatures will become invalid. If necessary, please re-apply for the issuance of the electronic certificate for signatures at the time of lifting the suspension.

Procedures for renewing My Number Cards for foreign nationals extending their period of stay / 在留期間延長に伴うマイナンバーカードの更新手続き

The expiration date of the My Number Card for foreign nationals holding a residence status with a fixed period of stay is set to coincide with the end of their stay. If your period of stay is extended, please complete the procedure to change the expiration date of your My Number Card before it expires.

NOTE:

  • No advance notice will be sent regarding the expiration of your My Number Card due to the end of your period of stay.
  • The expiration date of your My Number Card will not be automatically updated, even if you renew your Residence Card. If you do not complete the extension procedure before your My Number Card expires, the card will no longer be valid. There is a fee for reissuing the card. (1,000 yen in cash is required for the issuance of the My Number Card and the electronic certificate.)
  • Processing the extension of your My Number Card's validity period on the same day you receive your new Residence Card may take over an hour. Additionally, changing the expiration date during weekend office hours may not be available.
  • The expiration date of a My Number Card can only be changed up to the 10th birthday following the date of issuance (or the 5th birthday for those who were minors at the time of issuance).

  • Proof of identification* for the representative
    (* Please bring the same identification document as listed above under “Items to Bring When Receiving Your My Number Card”)

※You will need to enter the 4 digit user authentication certificate PIN for the My Number Card renewing.

For renewing My Number Cards for children under 15 years old

  • Child's My Number Card
  • Your family child's renewed Residence Card
  • Proof of identification* for the parent/guardian
    (* Please bring the same identification document as listed above under “Items to Bring When Receiving Your My Number Card”)
  • Documents confirming representative authority, such as a birth certificate with translation (Necessary only if your relationship with the child cannot be confirmed through the Matsudo City residence registration)

If you are currently applying for renewal and have not yet received your new residence card

If you do not receive permission to change your residence status or extend your period of stay by the expiration date printed on your My Number Card, you may apply for a two-month extension of the My Number Card as a special measure.
When applying for a special extension of your My Number Card, please provide proof that you have applied for a change of status or an extension of stay, in addition to the documents listed above.

A proof confirming that you have applied for VISA renewal

  • Residence Card with “Application for Renewal of Residence Status Pending” printed in the section for Residence Renewal Application Status at the bottom right of the reverse side
  • Completion Email for Online Application for Change of Period of Stay
  • Application receipt for extension of period of stay or change of residence status

※ Receipts or similar documents issued through proxy applications by administrative scriveners, etc. are not accepted.
※ You will need to renew the expiration date on your My Number Card again when you receive your renewed Residence Card.
※ There is absolutely no additional extension of the special two-month extension.

Other My Number Card Services Available / マイナンバーカードでできるその他の手続き

Obtain official certificates at convenience stores / 公的証明書のコンビニ交付サービス

My Number Cards can be used at convenience store multi-copy machines (FamilyMart, Lawson, 7-Eleven, etc.) to print official documents like residence certificates (juminhyo), seal registration certificates, and various tax certificates, etc.
Services are available from 6:30 AM to 11:00 PM (usually including holidays) using the card's 4-digit PIN.

【NEW!】Filing of National, Prefectural & Municipal Tax Returns Online / オンラインでの確定申告・市県民税申告

You can now use your My Number Card for convenient online filing of Japanese income tax returns (e-Tax) and municipal/prefectural tax returns (eL-Tax) via smartphone or computer, starting with 2026 filings for 2025 income. My Number Card enables secure identity verification using IC chip scanning, requiring a 4-digit PIN for authentication and a 6-16 digit signature password.
This also eliminates the need to submit paper documents for many procedures.
Ensure your card is valid and you know your passwords before beginning the online filing process.

After March 2022, If you have an Individual Number Card, foreign nationals (all the status of residence excluding "Diplomat" and "Temporary Visitor") can apply for residence procedures online.

No-writing Counter Services Using My Number Card / 書かない窓口システム

On January 31, 2025, the City began operating a “No-Writing Counter Service” using the government-promoted system.
The "No-Writing Counter Service" is a system that allows city hall visitors to complete various forms without writing them by hand at the counter by reading their My Number Cards.
The service reduces the time spent at the counter and waiting time by eliminating the need to fill out application forms. At the beginning, the service will be provided sequentially starting with the procedures that are available depending on the situations at each counter.

Use of a My Number Card as a Health Insurance Card / マイナンバーカードの健康保険証利用について

With the integration of the My Number card and the National Health Insurance Card card, the National Health Insurance Card has been discontinued after December 2, 2024.
To avoid the hassle of managing paper certificates, it is recommended that you transition to the My Number Card for health insurance. However, a "Certificate of Eligibility" is issued as a substitute for the traditional insurance card that can be used at medical institutions to ensure that you can still receive care.
